Insurance Information

Insurance/Payment Information:

We accept all major insurances, including United Healthcare, Medicare, Equality Care (T-19), Fiserv, Great West Life, and Tricare to mention a few.

Our physicians are participating in Blue Cross Blue Shield. This means that a formal agreement exists between our physicians and the insurance company regarding claims submission and processing, covered services, allowable charges and payment rates.

Out-of-pocket expenses (e.g. deductible and co-payment amounts are determined according to your benefit plan.

Obstetric Patients:

At the beginning of care you are given an estimate of total costs involved for your planned delivery. A deposit is required on your first visit with the doctors. You will be given this information when calling to schedule an appointment with our doctors.

Surgical Patients:

Surgical patients will be given an estimate of the surgery to be performed and are required to pay their deductible and a portion of their co-insurance prior to the procedure.

We understand that rising health care costs are a concern for everyone. We will gladly discuss a payment plan that is workable for you and our practice.
